Ordering a birth certificate from Northern Ireland

I live in England and need to order a birth certificate from Northern Ireland.

I have searched the Ireland - Civil Registration Index 1864 - 1958 and located the entry that I think pertains to my man but I'm at a bit of a loss as to what to do next, or even if I have all of the information I need.

The registration index has given me his name, the quarter and year, registration district, volume and page number. However, when I look at the application form on nidirect, it asks for the address the person was born at, the parents names and the mothers address.

Can anyone tell me where I find this additional information?
