you should order the death certificate direct from the General Register Office
http://www.gro.gov.uk/gro/content/certificates/

You will need to register first if you haven't ordered from them before, and you will need the volume/page details from the national indexes for John Pratley's death registration.
Quarter/Year - Dec 1911
volume number 11b
page number 146

If you want to get the certificate it will cost £9.25
Don't order it via Ancestry or other companies which charge you much more

I'm not sure about the burial. I can't see it anywhere online. Blaenau Gwent County Borough Council have the records for the cemetery, but say they charge a whopping fee of £22 for a burial search
