1705 Marriage Record, Chudobin / Lhota, Mlynář widow to Juračka

Hello.

I need help interpreting the bride data in the attached snip. According to the "super index", she was a widow, but I think the name given, Mlynář, is her first husband's name. I think the first husband might have been "Pavel". I also don't understand the significance of adding "horni" after the surname. I have observed this several times. I know it means "upper" or "over", but what does it mean in conjunction with a surname?

Her entry in the "super index" (attached):

The link for the date marriage record (attached):

I think that the groom was Tomaš Juračka, son of Vit of Lhota. My question is, what exactly does it say for the bride? These would be my 6th great grandparents.

Thanks,
Mary
