You can search the canadian archives for war veterns at http://www.collectionscanada.gc.ca/military-peace/index-e.ht...
just select which conflict you are interested in ( eg. ww1,ww2) and search by name you should be able to locate the "Particulars of Recruit" which lists some of the info you are looking for
