For Info on the hospital this may help:
There were Booth Memorial Hospitals in most major U.S. cities and they were run by the Salvation Army. The hospitals are closed now (maybe) and the records have been warehoused at the Salvation Army Facility in Des Plaines Illinois. This is their number (847) 294-2900.
You have to send a letter from a doctor (saying records are needed for medical purposes) and a check for $25.00. You have to give them your name, your birthmom's name, your date of birth and the city in which you were born.
