Hazel B. Katz, 59, of 1022 South Floyd Street, Louisville, Kentucky, died at 12:15 p.m. Tuesday, September 23, at the Methodist Hospital after a long illness. Mrs. Katz, was the mother of Dr. Kenneth J.S. DeSimone, of Greensburg and Campbellsville. Hazel B. Katz, was born on June 17, 1910 in Galena, Indiana and is survived by her husband Ralph Katz of Louisville and one son, Dr. Kenneth DeSimone. Survivors include her mother, Agnes Sirey, of New Albany, Indiana, four grandchildren, Lucretia DeSimone, Ralph DeSimone, Deke DeSimone and Annie Dee DeSimone. She is also survived by three sisters and six brothers, and a host of other friends and acquaintances. Funeral services will be conducted at the Arch Heady Funeral Home. 1201 East Oak Street, Louisville, on Friday morning (the exact time is pending at press time Wednesday). Burial will be in Evergreen Mausoleum.
