Obtaining birth certificate of living individual born in Ixelles

I'm hoping someone has had experience with obtaining a birth certificate from the city hall in Ixelles. I have contacted the embassy and received phone and address but no email contact at all. I want to get the forms needed to request a copy of a birth certificate and the proper procedures, costs and acceptable payment methods. I don't speak french or flemish and wondered if anyone has some words of wisdom for me
