Surname Traditions in Holland?

Surnames: van Cortenbosch, van Swaenswijck
Several of my early ancestors (14th to 17th centuries) hail from Holland, especially Noord-Brabant and Wijck in Gelderland. In two cases, I've found that someone's surname contained a city name in it: 'van SwaensWIJCK' (Wijck) and 'van CortenBOSCH' ('s-Hertogenbosch, often called simply Den Bosch or Bosch). Does this mean anything? How was a surname typically picked in Holland?
