Surnames: Haynie Garcia
My now deceased step-father was long assumed to have taken an alias and to be on the run. I ordered an SS-5 and received a letter from them that there was no SS-5 but there was an OAC-760. The form makes it even more suspect that this is not my step father but rather a stolen identity.
Name - Franklin Warren Haynie
Birth date 3/21/19 in Loas Gatos, CA (My step father stated that he was born 3/21/12 in Louisiana)
Father Jon Walter Haynie
Mother Alicia Louise Garcia
Says he was a trucker in Chicago. There is also a date on the OAC-760 on the lower right hand side of the form but I cannot make out what the date references - DOD? Date of benefit? The SS number starts with 728 so it was issued to a railroad worker.
Any help would be appreciated. Help me!! :)
