I came across a tombstone with the following inscription
SAMUEL KARPIAK
1883-1921
This was in St. Mary's Ukranian (Rusyn) Cemetery in Elkins Park, Pennsylvania. The caretaker there stated that the cemetery records were in the possession of the Ukranian Cathedral in Philadelphia, Pennsylvania.

I don't know of any family connection but thought it might be helpful for someone.
