Thought this would be a good spot for people looking for help on a government form (i.e. naturalization documents, death certificates, etc).

Often the scan you need for your tree is difficult to read / incomplete, if you need help finding a template or can provide others with help, please post here.

One example - I have a partial and poor quality scan of the 1911 "Facts for Petition for Naturalization" for my great grandfather.

As far as I can tell this is a pretty hard-to-find document, I only found one other text-only version on the internet. I used the text along with my scan to create a clean blank template which has helped me to translate his answers.

If anyone has the same problem please let me know and I'll send the blank reproduction of this document to you.
