How do I obtain the birth certificate for Herbert Leeroy Manley or Fry or...2 Jan 1922, Sabula, Missouri

Surnames: Manley, Yarbrouch, Ralph, Fry or perhaps something else
How do I obtain a birth certificate for my father Herbert Leeroy Manley, born 2 Jan 1922 in Sabula, Iron County, Missouri to Annie or Anna Estella Yarbrough-Ralph-Fry or perhaps something else.

I don't believe my grandmother was married to my dad's father, so she was perhaps using her maiden name of Yarbrough although by then she had married and had children under the last name of Ralph. In the 1930 census, my dad is listed as Herbert Fry (from Estella's third husband) from whom she was already divorced. (there are a number of other husbands also.) Mr Fry was not the father, but a step-father.

I've contacted the Iron County Health Department, but they are unable to look up the record without a last name, which I just don't have.

You can order the birth record from the state for $15.00. There's a PDF form to request the record. There's even a line on the form that asks if the birth may be recorded under another name.
