Hello,

Could someone please advise me how to follow the steps in ordering a Death Certificate for my grandfather Kenneth Green O'Briant, Sr. when he died prior to the issuance of a Social Security Number.

Death Cert#: 8024
Birth: 17 Mar 1904 Ethel, near Kosciusko, Attala Co., MS
Death: 26 Apr 1948 Pensacola, Escambia Co., FL
Interment: 30 Apr 1948 Barrancas Natl Cemetery

He was an Officer for the US Navy at the time in 1948 stationed in Pensacola, Escambia Co., FL and buried at Barrancas Natl Cemetery.

I've gone through the Ancestry.com procedure but when I come upon the page requesting Social Security Number (must have) I've entered xxx-xx-xxx and 000-00-0000 and have not been able to get pass this screen without a message requesting a valid number.

Sincerely,
Catherine O'Briant

The newspaper article I have says unidentified man found murdered was later identified as Chief Hospital Corpsman Kenneth Green O'Briant, US Navy while on furlough.

First of all, don't order a death certificate through Ancestry. You can get one directly from Florida for about 1/4 the price. You don't need a Social Security number to get a death certificate from Florida.

Cost is $5. Because it is more than 50 years old, you will get cause of death.

Here is the link.

I usually just send them a short letter, with name, date, place and include a check for $5. I get the certificate in a few weeks.

Address is:

STATE OFFICE OF VITAL STATISTICS
ATTN: CLIENT SERVICES
P.O. BOX 210
Jacksonville, FL 32231-0042

There is a form you can use here:

http://www.doh.state.fl.us/planning_eval/vital_statistics/DH...

I usually just send a letter with my name, address and phone #, along with name, exact date of death, county and file # if I have it. If death is more than 50 years ago, you will get cause of death. If it is less than 50 years, ago, you can get cause of death, but will have to provide relationship and photocopy of your driver's license. If less than 50 years, I would use the form so they can easily see what you need.
