Name spelled 2 ways on same document (American Volunteer Soldiers, Mexican War, 1846-1848

I have run across several Reverence Cards & Reference Envelopes that have the soldier's name spelled one way at the top and a different way farther down the document. I have been using the first spelling. Is this correct? Should I put both spellings?

OT: These documents are facinating. Horses stolen by the enemy. Soldiers getting demoted (wish they had the full story on that one)

Use the name at the top. There is no field for alias names.
