Which rank to key in American Volunteer Soldiers, Mexican War?

In one place instructions are to key the highest rank, and in another it says to key the rank shown at the lower right of the name on the Record Card/Envelope. But in the case of a demoted soldier, the highest rank may appear on the lower LEFT.
So which do we use, highest rank or lower right rank?

I changed the wiki to show as keying the rank on the right side. It could definitely be a lower rank when the soldier has been demoted.
