Print Hierarchical Place Name List?

Does anyone know how I might print out a fully expanded, hierarchical, place name list? Ideally, I'd like to print just the place names without the associated individuals.

Or, I'd settle for a way to export such a list to Excel, or in a .PDF format.

I hope someone answers this as I need the same thing. The Places report is lame, doesn't allow any formatting or sorting either. Exporting most reports to Excel is also poor, as you can't get everything on one row. I, however, would like to have a report like you described with the option of WITH and WITHOUT individuals listed.
