Minnesota SSAH records - images with overlying attachments

I've copied my post from the US specific board as I haven't had a reply in over a week to this problem on there.

Very interesting project to index but have run into the problem illustrated with the images below.

I have a series of images where pages of information have been attached to the original records as a type of continuation of the record. On each successive record a layer is folded back until the original documents are revealed.

How should I index these pages.

Many Thanks

I would just key them as normal.

Reply appreciated but was is normal! The name on the overlay has no admission/ discharge date attached or any demographic data, just medical details. Should I key just the name? And the page that is duplicated on successive images should I key the details again.

Yes, if there are no dates etc I would just key the name, and I would key it again on the other images as they are not duplicates of the entire image.

Just confirming that we do key these.

The Q/A I have was Fwd'd me from Karen.

After receiving this, Karen uploaded the below image to the wiki.

Anna,

I can’t seem to locate an answer for this. Do we key names only on these forms, or classify as a cover page as they are not actual admittance forms?

Regards,

Karen

Karen,

We would key this record – any record that has the patients name.

Anna
