Hello everyone. I have a very dark and bad copy of my GG Grandmother's Civil War Widow's Pension card, or sheet. It is so dark that we can barely make out her name, let alone the small type beside her name.

Does anyone know of a site where I can download a blank copy of this form so I can find out what the different sections say?
