Also, in regard to that death certificate for Felix POTAPA. Felix's father's surname as written on the death certificate is, at first, PATAWSKI then lined through and re-written POTAPA. Weird.
The "informant" documented on this death certificate is either a friend or an in-law. I would assume the informant to be the person who's giving the information about the deceased to the county or the undertaker?
Felix's mother's name was Maria PROKOP
