The one who died in 1913 at 48 seems the most plausible.

I haven't obtained the actual record yet because:

A) Even the index was hard to locate in NY
B) You have to pay $$ for actual copies, and without any certainty I was much less eager to pay.

However, I plan on ordering a copy of that one soon.
