Everything matches but surname

For over 20 years we have been looking for my maternal grandfather. He left a note stating exactly where he was from, a small village in the Black Forest. Yet he did not appear in any of the records, nor could I find a matching family. Finally I found a death record that matched his mother in a nearby village. However her husband had a different surname. When I researched this family with "the different father" it matched exactly. Further, the youngest son had a birthdate 2 months earlier than the one we were told. This son arrived in New York on the exact date my grandfather said he did. I am not sure how to proceed. Apparently in the US he completely changed his first and last names. Has anyone run into this?
