I had two responses that both translated the surname as HOTEWEK.
After the document was translated,(combined with a few translated letters written in Russian from 1950's from Lviv,Ukraine) I discovered the mother's name was Madzej & that Hotewek was a surname from the pevious generation.
It is interesting that the name makes no sense in Ruthenian/Slavic-- I had a feeling something was amiss-- in my research I havent been able to find that name even once.
