Can anyone tell me if the name on the application is the name they put on the card, or can it be different? My grnadfather's name on the SSDI lists his first name & middle initial, I'm wondering if that's how his name is listed on the application too, or if it might have his full name? The main thing I'm looking for is proof of his middle name, so I don't know if it'd be worth it to apply for the application just for the rest of the info on it.
