I would appreciate any help to identify a document found among my deceased Grandfather's personal items. I have been able to translate sections of the document but not enough to tell me what it is.

This document has the name and birthdate of my Great Grandfather, Anton Scheiba 5 July 1887 and the location of Elbing which I know he lived at one time.

Very little was allowed to be discussed about my Grandpa's life in Germany. I would greatly appreciate any help you can provide.
