What does a Hawaii "Certification of Live Birth" look like?

There has been some recent controversy that Barack Obama was not actually born in Hawaii but overseas thus making him ineligible for President.
He has not released his actual birth certificate but has released this "Certification of Live Birth":

My question is if this is supposed to be an official state document it should include the name of the responsible state official as well as that official's signature. All the other cases of these certifications of birth issued by a state I've seen include the state seal and the signature of a registrar of records.
Does the "Certification of Live Birth" in Hawaii not include such a signature?
