Caro - I agree with the format, but your explanation isn't quite right - I was born before 1969 but I'm not 50 yet!

There are restrictions on ordering certificates for deaths in the last 50 years if the person who died was aged 16 or under (which is why you have to enter the age at death), but for all other deaths, right up to 2006, when you get to the particulars page all you have to do is supply the index entry details.

The requirement to enter the month and year for a death in 1985 rather than the quarter and year is because this is what was recorded in the death indexes from 1984 onwards.

This is all gleaned from experience of ordering many death certificates as part of a one name study, and it was hard work wringing the relevant information out of the GRO staff, I can tell you...
