From 1 April 1969 the format of birth certificates changed a fair bit. It was at this point that the forename(s) *and surname* of the child were first entered on the certificate itself. Prior to that, only the forename appears in the Name column, and the surname which is shown in the index was inferred from the father's name (or mother's where illegitimate). Where there was ambiguity about the surname - for instance if a putative father's details were entered on the certificate, or the mother was using more than one name - two index entries would be created, even though it was a single registration and a single certificate.
