Yes, this is not unknown. Until the 1960s births were registered with a forename only, with the surname in the index being derived from the surname of the father (if the parents were married), or the mother (if not). In the case of a child whose parents were not married but whose father acknowledged paternity and accompanied the mother to register the birth and sign the register, the father would be named in the Father's Name column and the registration then appears twice in the index, once under the mother's surname and once under the father's.
