FTM2011 gives you the option to put the data in columns and names in rows, or the names in columns and data in rows. In fact, you're not limited to name in rows.

This is after you click:

Share button > csv > rows or columns?

And that INCLUDES column headings.

If your version doesn't do that, you might consider upgrading to FTM2011.
