I have used a combination of FTM12 and www.blurb.com. You can see a sample of my book here:

Many charts and reports can be created from FTM12 from a surname report to an individual history with all the notes and sources. However, I prefer to have more flexibility in placing images and creating historical context around some ancestors, including maps. For this, blurb is really useful - I used their BookSmart product.

Another advantage of using blurb: you can create a hard or softbound book, an electronic version and/or a pdf.
