It looks like indexes to me, but there is no index type to select. Therefore, should use the Gazette Page With Names type and key them all?"
An Index would just have their name and page number (not their location) and you would change the form type for Index to Cover Page.
The one you have needs to be keyed but don't worry about the number before the name. If you don't have the first page you would just key page number, name & locality leaving the event and date fields blank/empty. (they are actually lists of unclaimed letters which we come across periodically)
