Common Review Mistakes in Ottawa Journal

I've started reviewing the Ottawa Journal sets and I am finding two common mistakes:

1. For Death Notices, if there is a maiden name shown for
a deceased woman, we are supposed to put the maiden name
in the Surname category and the married name is reported
under the Spouse name.

2. According to the field helps, if a year is not mentioned,
we are to key the year that appears in the banner at
the top of the page, right hand corner.

Just trying to help the accuracy!
