Please watch for different form types.
The death record day/month/year is NOT always going to be in the same place at the top of the form. It all depends on the form that was used.
Many times there is more than one date on the form. If there is, usually, the very first date listed is the date the record was recorded in the book. You must look further down in the record for the actual death record day/month/year (aka date of death). Sometimes the recorded date and the date of death can differ by years.
Do not assume the fathers surname if it is not listed on the page. Leave the field blank.
If there is a middle name listed it should be entered in the Given name field following the Given name.
(Attached is an example from an Extended Handwritten form and Atti di Morte - Parte II- Serie A form with two dates. The second date is the date that should be used when keying the record.)
