Hi, Audrey:
The date at the top is only used when no dates appear with the names. Since this example has dates, you can ignore the date at the top. Remember to use the later date (in parentheses) for the military date. Here's what the project instructions say:

"Military dates include, but are not limited to, enlistment dates, service dates, dates of rank, and retirement dates. When multiple dates appear for an individual key from the latest date. ... When there is a list of names without dates, use the date from the header. If there isn't a date in the header, leave the date fields empty."
