US Southern Claims -Mulitple questions

Not sure how to handle this one.
This is the first page in this image set.
1. This image is clearly not the first page of testimony as it has the #5 written at the top of the page, but it is testimony and continues from previous pages. Just not sure if it is testimony from a claimant or from witnesses-is there any way to differentiate testimony between claimants and witnesses? So would I still select Testimony First Page as the image type? If no, than what would be the correct image type to select?
2. The document clearly lists Amey M. Adlington as a witness. But is he/she just a witness for the mark of Guy Small? Is Guy Small also a witness? (witness is clearly written in the singular, not plural). If Adlington is just a witness for the mark of Guy, is Guy then the claimant?

3. On the second attachment which is still part of the first page of this image set, James Roach states that he was "born in Sav. a slave and was set free when Sherman's army came here". To me it seems that Sav would be keyed as residence from this statement. Opinion? Realize that Sav is Savannah, but will key as seen.
