I can't seem to find the answer to this. I'm seeing signatures for some workers, do I use the signature for the name if there is a difference from the name given in the first column? Some have intitials for their given name, but in the signature there is a full name.


Also, under the list of names there are a few names, but they look like company names instead of workers, how do I enter these? Will attach a pic.
