RI vital extracts - newspapers; prefixes

I hope it's okay to post RI newspaper extracts here too as they have basically the same instructions as the other two RI vital extracts. In the instructions it states that "if child of Robert Palmer" appears that we are to key child of in the prefix field and Robert Palmer for the father given and surnames. Does this go for daughter of, widow of, son of, etc? Also, if a record states Mrs Eliza Dunn, wife of Robert, daughter of Charles Winfrey (for example)would the prefix field be as follows?
Mrs wife of daughter of (if not how would it be indexed)
Also, I have been adding the surname to the spouse and parent when it is not listed, as in the above case with Robert. Have I been doing this correctly. I believe the surname fields for the various individuals (spouse, father, mother, spouses parents say something different as opposed to all being consistent). I apologize if I have been doing this in error.
